GP+ is a pathway for primary care teams. It sits alongside routine care rather than adding to it — a short capability prompt, a review the person completes themselves, and one clear next step they can actually keep. Not a diagnostic tool, and never a replacement for clinical judgement.
How it runs
Capability decline rarely arrives as a presenting complaint. GP+ gives a short, non-clinical prompt set that surfaces it during contact you are already having.
The person completes their own review in their own time. The picture belongs to them — the practice sees only what they choose to share.
One meaningful action, not a list. Usually the 30-Day Capability Challenge, sometimes a strength or bone-health pathway, sometimes a conversation.
Capability is re-reviewed over time, so change is visible to the person and to the team supporting them.
