My Path

Pathways / GP+

Notice capability earlier.
Act with authority.

GP+ is a pathway for primary care teams. It sits alongside routine care rather than adding to it — a short capability prompt, a review the person completes themselves, and one clear next step they can actually keep. Not a diagnostic tool, and never a replacement for clinical judgement.

How it runs

Four stages, no new clinic.

01

Notice

Capability decline rarely arrives as a presenting complaint. GP+ gives a short, non-clinical prompt set that surfaces it during contact you are already having.

02

Understand

The person completes their own review in their own time. The picture belongs to them — the practice sees only what they choose to share.

03

Act

One meaningful action, not a list. Usually the 30-Day Capability Challenge, sometimes a strength or bone-health pathway, sometimes a conversation.

04

Sustain

Capability is re-reviewed over time, so change is visible to the person and to the team supporting them.

Where it fits

Inside contact you already have.

Long-term condition reviews

Add a capability lens to reviews already in the calendar.

Post-fracture and falls follow-up

Turn a single event into a sustained practice.

Frailty and over-65 checks

A non-judgmental picture that people are willing to complete.

Social prescribing

A structured destination for referrals, with a clear first action.

Health checks

A capability question alongside the cardiovascular one.

Practice populations

Anonymised, consented patterns — never individual surveillance.

What the person keeps

Agency stays with them.

The review, the snapshot and the reflections belong to the member. Practices see only what a person consents to share. GP+ is designed to strengthen the person's own capacity to understand and act — not to create another record they are measured against.
